The formation of the division followed two subsequent trainings of First aid and Home based care. The first cohort of 32 members trained in First aid in 2010 and later 13 more members trained in Home Based Care as part of the First aid and Home based care project sponsored by Johannitter Germany.

The division is a product of a Life Saver International First Aid Course (LIFAC) and Home Based Care (HBC) project sponsored by Johannitter Germany. To change the obstinate nature of some Ugandans in embracing voluntarism, First Aid and Home Based Care training was conducted to 30 people in Gayaza and its neighborhood. The enthusiasm that came with the successful completion of the training eventually led to the formation of Gayaza division in 2010.

The division was formed in 2011 following Life Saver International First Aid Course (LIFAC) and Home Based Care (HBC) training to 27 members in Bunamwaya with support from Johannitter Germany. The beneficiaries valuing the imprescriptible knowledge gained were inspired to form a division which has become an indefatigable force for St John in Bunamwaya.

Between 1990 and 1992 it had been realized that St. John Ambulance was not popular like other voluntary organizations in the region but only heard of or confined to Kilembe copper mines limited - safety department.

Kampala headquarter division is the oldest active division formed upon the completion of head quarter construction in 1960. The division is housed at st.john headquarters which has always made it possible to have a readily prepared emergency rescue team to respond to emergencies within and outside the capital.

Mulago Nursing division was started by Dr. Phillip Mitchell (the then medical superintendant of Mulago hospital) in 1937. Dr.Mitchell and Dr. Owen conducted the first ever first aid training to Africans and the success eventually led to the formation of Mulago division. Later, due to political instability that swayed the country in the subsequent years, it ceased to exist but later re-opened in 2001with 30 members. The division has been the model in smartness and very active in emergencies and public duties. The patron is Talwana Augustine, M.B.E.

The division was started in 2008 after a comprehensive first aid training to 25 people from the headquarters.  The spirit of voluntarism soon evolved amongst the beneficiaries and the certitude to serve human kind eventually led to the formation of St.John division in Nansana. The division has since established strong partnership with the Ministry of health, community based organisations and faith based institutions among others which has seen St.John featuring in most of the community based health initiatives in Nansana and its neighborhoods.

The formation of Hoima division followed a First aid training for 50 members in Hoima division in 2004 at Koob hospital. The division covers Hoima, Masindi and Kibaale district in the western region of the country.

Kagoma Maganjo division was established in 2002 after a successful first aid training to over 40 community members from the head quarters. In 2003, another cohort of 14 people was trained in First aid. The branch was incepted through the efforts of community members in Kagoma Maganjo with support from the head office.

Kasasa division was started by John Kirkwood in 1986 and the first aid course was conducted by a Kenyan counterpart Munungi Musee. The main benefeciaries of this division are the students of St.Charles Lwanga s.s.s Kasasa, and the neighbouring communities. The division is challenged by the school environment its operating in - as most of its members are students who retire from service on completion of their studies.
